Brace Yourself
If you are not yet enrolled in Medicare, you may have individual or group health insurance and you probably have a fairly good idea of what medical services are covered and how your insurance works. Brace yourself! You are about to enter the world of Medicare, a world of far greater complexity, full of important decisions right when you wish life could be a little easier. Medicare is not at all like insurance you are used to.
An Unwelcome Surprise
Just one glimpse at some of the surprises you will find will amaze you. If you are still working when you become eligible for Medicare, you may have heard that you don’t have to sign up for Medicare Part B medical coverage until after you stop working. You elect not to take Part B and later have some medical expenses.
Imagine your initial shock and dismay, when you find out that you have to pay eighty percent (80%) of the bill for those medical expenses. What happened? You were working for a small company with fewer than 20 employees, Medicare was primary, and your insurance company assumed you had Medicare Part A and Part B even if you didn’t. They will only pay their twenty percent (20%) share. Find out what other surprises await you.
Locked Out
Your spouse is over 65, or you have worked past age 65 and you elect to take COBRA coverage putting off Medicare Part B enrollment until later. The Initial Enrollment Period for Medicare Part B is eight months from when you quit work or lose insurance coverage, whichever occurs first. You think you are covered by insurance until you find out that Medicare doesn’t consider COBRA as coverage for purposes of this eligibility rule. The same is true if you change your insurance plan after 65 and have a gap in coverage, which could happen if you change jobs. You coverage ends when the insurance you were covered by at age 65 ends, not counting any other following insurance or COBRA.
Now you quit work, or your COBRA ends, and you want to enroll in Medicare Part B. You listen in disbelief, when you are told you have to wait until the next General Enrollment Period possibly delaying coverage for up to 15 months and pay a 10% penalty every year. You then call an insurance agent looking for something to cover you until then, only to find out you are in real trouble. The agent can’t sell you a Medicare Supplement unless you already have both Medicare Part A and Part B.
Just When You Were Having Fun
Here is another surprise. If you take a cruise, say to the Alaska, you are covered by Medicare, and the ship is registered to a foreign country, you may not be covered by Medicare. You will be covered only if you meet certain conditions or if you have the right Medicare supplemental coverage, but only to a lifetime maximum of $50,000 and with no provision to bring you back home for medical treatment.
